Decision makers will make inappropriate decisions that can adversely affect the product, process, procedure or system. Consequently uncontrolled risks may be introduced due to poor decisions throughout the life cycle of the system and potential system accident.
Safety professionals work in imperfect entities, and they may send excessive time and resources attempting to meet an inappropriate program objective and/or fixing an inadequate complex design, or an existing process, project, or facility. This may be due to poor initial decisions. Consider that the decision-makers may not have understood safety axioms. We will discuss the safety axioms we should apply when making safety-related decisions. Also, what follows is a discussion of some common statements that may reflect a limited background, knowledge, experience, attitude, perception, misguidance or arrogance, along with counter statements that support a more logical safety-related argument.
