A case management tool kit that includes resources and a level of care decision tool for case management is provided to attendees. The tools are used to address the appropriate level of care for a patient in a scenario
Why should you attend: Little guidance is available for the rehabilitation and community integration portion of the continuum of care once a medical model of brain injury services is no longer appropriate. We are in need a some type of guidance, across the board, for case managers, discharge planners and other coordinators of care to use when individuals with TBI are preparing to leave the hospital to re-enter the community and workforce or are already out there and struggling for reasons no one can quite put their finger on.
The absence of consistent guidance or direction for case managers leads to inconsistencies of care between individuals, between areas of the country, and between physician disciplines. It makes it difficult for payer sources to know what the right thing to do actually is. Resolution of these inconsistencies is one goal of the case management care path we will review today…a care path for the individual with moderate to severe TBI who is preparing to structure their life and the life of their family members to live with residual injury.
